Job Description Job Description QUALIFICATIONS: Must meet the
criteria for a Qualified Mental Health Professional (QMHP) under
DHS Rule 132.25. Must possess skills required to provide person
centered, mental health clinical assessment; certify diagnosis;
recommend treatment; conduct treatment planning as well as
individual and group therapy/counseling; and provide case
management and coordination. Minimum of 24 months of continuous
sobriety required for persons recovering from a substance use
disorder. Must meet Illinois Health Care Worker Registry and
criminal background check requirements. Bilingual preferred.
RESPONSIBILITIES AND DUTIES: Mental Health Assessments /Treatment
Recommendations Conduct face-to-face, person centered mental health
assessments that result in the identification of the current MH
diagnosis and recommendations for treatment services to address the
problems associated with that diagnosis. This process includes, but
is not limited to: Gathering information needed to provide a mental
health diagnosis including: i) Reason for seeking treatment and
current symptoms. ii) History of mental health symptoms and
treatment, including psychotropic medication history. iii) History
of abuse or trauma including childhood sexual or physical abuse,
intimate partner violence, sexual assault. iv) Family history,
including mental illness v) Physical health, Social adjustment,
daily living skills, and environmental threats. vi) Legal history
including guardianship and current court involvement. vii) Use of
substances, both historical and current. viii)Evaluating collateral
reports on current/previous symptoms, medications, treatment, and
responses b) Completing Mental Status Exam. c) Identify Current MH
or Co-Occurring Disorder Diagnosis d) Entering information into IM
CANS per state guidelines. Treatment Planning Collaborate and
assist persons served and their families in the development,
review, and modification of their individual treatment plan based
on the mental health assessment and ongoing reviews of treatment
progress, medication compliance and other areas of life
improvement. Therapy/Counseling Provide individual counseling
interventions based on therapy techniques promoting emotional,
cognitive, behavioral or psychological changes and recognized as
evidence based and/or best practices. Case Management. Provide case
management assessment, planning, coordination, and advocacy
services for individuals who need multiple services and require
assistance in gaining access to and in using mental health, social,
vocational, educational, housing, public income entitlements, and
other community services to support the individual in the
community. Client-Centered Consultation a) Engage in
client-centered consultation with Stepping Stones staff or others
who are providing services to the client, offering or obtaining a
professional opinion regarding their current functioning or the
improvement thereof. b) Discuss treatment progress, adjusting
current treatment, or addressing the need for additional or
alternative mental health services. Psychotropic Medication
Training / Psychotropic Medication Monitoring a) Provide clients
with education regarding uses, side-effects and warnings regarding
psychotropic medications prescribed to them by in-house or external
providers b) Monitor client compliance with medication through
client self-report and clinician observation. c) Address concerns
regarding client medication issues with client and report concerns
to the medication provider. Provide consultation/supervision for
staff on mental health/family issues. Provide crisis intervention
services as needed according to policy and procedure. Use the DHS
Firearm Owner’s Identification (FOID) reporting system to notify
DHS within 24 hours when individual is determined to be a “Clear
and Present Danger” to themselves or others. Complete all
documentation in a timely manner a) Mental health clinical files.
b) Incident reports. c) Referrals for nursing or other services. d)
Staff service reports. e) Logging of services in Client Tracking
database. f) Other documentation as required. Meetings,
In-Services, Conferences, and Committees a) Attend and/or present
in-services or workshops as scheduled. b) Attend and/or present
staff meetings as scheduled. c) Participate in various
organizational committees as requested. Other responsibilities as
