Job Description:

Encompass Health is seeking a dedicated Licensed Practical Nurse (LPN) to join our compassionate team, focused on providing high-quality rehabilitation and home health/hospice services. In this role, the LPN will work closely with an interdisciplinary team to deliver patient-centered care, aiding in our patients' recovery and support during critical times. This position is pivotal in helping patients regain their independence after experiencing serious health challenges. Administer nursing care to ill, injured, or disabled patients within a rehabilitation environment or home health setting Monitor and report changes in patient symptoms or behavior Communicate with collaborating physicians or specialists regarding patient care Educate patients about health maintenance and disease prevention Facilitate patient rehabilitation and provide emotional support to patients and their families Maintain accurate patient medical records and operate medical equipment safely Strong interpersonal and communication skills Proficiency in maintaining accurate and detailed medical records Capable of conducting physical examinations and interpreting medical tests Knowledge of administering medications and proper dosages Ability to work effectively in a team and independently with minimal supervision Medical Insurance Dental Insurance Vision Insurance 401k Current LPN license in the state of practice CPR certification
